Refill Request Form

Terms of Use
By submitting a request you acknowledge that you have read and agree to the following terms:

1. You acknowledge that you are 18 years of age or older.

2. Our online refill request form is for current patients only (your pet MUST have been seen by one of our doctors within the past year).

3. Advance notice requirements: Immediate requests cannot be filled. Minimum 24 business hours required on all orders. For example, if you submit a request at 3 p.m. on Friday, your order will not be ready until 3 p.m. the following Monday (as long as it is not a holiday).

4. Specialty Service notices: The Internal Medicine Department is closed on Fridays–orders submitted on Thursday will be ready on Monday. The Surgery Department is closed on Wednesdays–orders submitted on Tuesday will be ready on Thursday.

5. Payment policy:
–For VESH pick-up orders, payment is due upon receipt at our hospital.
–For home delivery orders, payment is due prior to mailing. A staff member from our hospital will call you at the telephone number provided below to confirm your home address and to process payment prior to mailing your order. Please note an additional $5 fee is added to home delivery orders to cover postage and handling.
